Welcome to ThriveWell Men’s Health Clinic (“ThriveWell,” “we,” “us,” or “our”). These
Terms & Conditions (“Terms”) govern your use of thrivewelltrt.com (the “Site”) and the
telemedicine services we provide. By using the Site or enrolling in our services, you
agree to these Terms.
1.1 About Our Services: ThriveWell is a cash-pay telemedicine practice offering
testosterone replacement therapy (TRT), medical weight loss (GLP-1/GIP medications),
peptide therapy, vitamin injections, and diagnostic labwork to eligible patients. We are
currently licensed to provide care to patients located in Ohio and South Carolina, with
additional state licensure pending. Services are provided by licensed nurse practitioners
under appropriate collaborative physician agreements.
1.2 Eligibility: You must be at least 18 years old and physically located in a state where
ThriveWell is licensed to practice at the time of your visit. You must provide accurate,
complete health information; withholding or falsifying information may result in denial or
discontinuation of care.
1.3 Not for Emergencies: ThriveWell is not an emergency medical service. If you are
experiencing a medical emergency, call 911 or go to your nearest emergency room.
1.4 Clinical Discretion: All prescriptions, dosing, and treatment plans are determined
solely by the treating provider based on clinical evaluation, lab results, and applicable
medical standards. Enrollment in a program or payment of a fee does not guarantee a
prescription will be issued.
1.5 Payments & Billing: Services are cash-pay; we do not bill insurance.
Subscription/membership programs (e.g., TRT, GLP-1, ThriveWell Elite) are billed on
a recurring basis (monthly or as otherwise disclosed at signup) until cancelled.
You authorize ThriveWell to charge your payment method on file for recurring fees,
medication costs, and applicable lab or visit fees.

1.6 Medication & Shipping: Medications are dispensed through licensed third-party
pharmacy partners and shipped directly to you. Delivery timing may vary. You are
responsible for providing an accurate shipping address and for safe storage and
handling of medications once received.
1.7 Patient Responsibilities: You agree to provide truthful health information, follow
your treatment plan, attend scheduled check-ins and lab draws as required for ongoing
prescribing, and promptly report side effects or health changes to your provider.
1.8 Limitation of Liability To the fullest extent permitted by law, ThriveWell and its
providers are not liable for indirect, incidental, or consequential damages arising from
use of the Site or services. Nothing in these Terms limits liability that cannot be limited
under applicable law.
1.9 Intellectual Property: All content on the Site (text, graphics, logos) is the property
of ThriveWell and may not be copied or used without permission. Product and Service pricing may not be copied or shared without our written consent. 
1.10 Changes to These Terms:  We may update these Terms from time to time.
Continued use of the Site or services after changes constitutes acceptance of the
revised Terms.
1.11 Governing Law: These Terms are governed by the laws of the State of Ohio, without
regard to conflict-of-law principles.
